Публічні механізми трансформації та розвитку сфери охорони здоров’я України

Abstract

У дисертаційній роботі обґрунтовано теоретико-методичні підходи та розроблено науково-практичні рекомендації щодо реалізації публічних механізмів трансформацій та розвитку охорони здоров’я України (нормативноправового, інституційного, фінансового, кадрового механізмів, механізму надання електронних послуг) в умовах сталого розвитку та викликів сучасності. У першому розділі узагальнено механізми публічного управління у сфері охорони здоров’я, які реалізовано в іноземній практиці (фінансовий, інформаційний, інституційний, організаційний механізми) та вітчизняних реаліях (інституційний, економічний, організаційний, правовий, фінансовий, інформаційний, кадровий, механізм державно-приватного партнерства). Визначено базовий механізм реалізації публічної політики в зазначеній галузі (інституційний, організаційний, економічний, кадровий, фінансовий, інформаційний, правовий, публічно-приватного партнерства) з урахуванням глобальних викликів, пандемічного загострення та євроінтеграційних домовленостей, реалізація якого має забезпечуватись такими інструментами публічних трансформацій, як технологія блокчейн, інформаційно-комунікаційні інструменти, відповідно до означених напрямів: ефективна й доступна система охорони здоров'я, підвищення якості та безпеки медичної допомоги, розвиток первинної медичної допомоги, поєднання державно-приватного забезпечення та медичного страхування. У другому розділі роботи узагальнено методологічні підходи до класифікації законодавства у сфері охорони здоров’я, нормативно-правової бази реформування сфери охорони здоров’я України. Запропоновано механізм нормативно-правового забезпечення розвитку охорони здоров’я, який включає міжнародний, національний та регіональний рівні. Визначено принципи реалізації механізму нормативно-правового забезпечення розвитку охорони здоров’я та основні стратегічні вектори забезпечення високоякісної та ефективної державної політики України у сфері охорони здоров’я, які дадуть змогу адаптувати програмні документи сфери охорони здоров’я Цілям сталого розвитку. Удосконалено інституційний механізм державного управління системою охорони здоров'я на основі визначення основних функцій, завдань, організації діяльності міжнародних, національних та регіональних інститутів державної політики, інструментів (нормативно-правові, фінансові, економічні, інформаційно-комунікаційні, дослідження та розробки, освітні та просвітницькі) та методів (організаційні, економічні, психологічні, спеціальні, адміністративні, цифрові технології) їх реалізації на центральному та регіональному рівнях управління. Дослідження фінансового забезпечення підтримки медичної галузі в умовах пандемії дало можливість виділити ключові проблеми фінансування галузі. Для підвищення ефективності використання бюджетних ресурсів на різних рівнях державного управління та забезпечення сталого розвитку сфери охорони здоров’я виділено напрями трансформацій публічних фінансових механізмів забезпечення сфери охорони здоров’я, реалізація яких можлива відповідно до принципів універсальності, справедливості, прозорості, підзвітності, ефективності, вільного вибору, конкуренції постачальників, субсидіарності та фінансового захисту. У третьому розділі систематизація міжнародного досвіду забезпечення систем охорони здоров’я на основі індикаторів (громадське здоров’я, доступ до лікарських засобів, надання послуг в охороні здоров’я та фінансування, кадрові ресурси системи охорони здоров’я) та рівня розвитку системи охорони здоров’я за міжнародними рейтингами доводить, що стратегічними цілями функціонування всіх систем охорони здоров’я є забезпечення спроможної системи громадського здоров’я, доступності і якості медичних послуг. Порівняльний аналіз результатів упровадження публічних механізмів у сферу охорони здоров’я в умовах пандемії в Румунії, Молдові, Угорщині, Словаччині, Польщі, Україні дав змогу визначити фактори ризику сфери охорони здоров’я та ключові пріоритети сфери охорони здоров’я. Спрогнозовано адаптовані індикатори досягнення Цілі 3 Цілей сталого розвитку в Україні, регламентованих у Національній доповіді «Цілі сталого розвитку: Україна на період до 2030 року», за допомогою статистичних функцій Excel, в основу яких покладена лінійна регресія. Доведено необхідність реалізації публічних механізмів трансформації сфери охорони здоров’я, що відповідають викликам сучасності. З урахуванням основ стратегії реформування медичної галузі, стратегічних змін діяльності уряду в умовах пандемії та повномасштабного військового вторгнення Російської Федерації на територію України визначено трансформаційні напрями публічних механізмів розвитку системи охорони здоров’я в умовах реформування, пандемії та воєнного стану, реалізація яких забезпечить досягнення сталого розвитку сфери охорони здоров’я України. Запропоновано алгоритм дій щодо забезпечення спроможної системи громадського здоров’я. У контексті трансформації публічних механізмів регулювання сфери охорони здоров’я на місцевому рівні виділено пріоритетні напрями для залучення громади до трансформації публічних механізмів регулювання охорони здоров’я в умовах децентралізації України. Розроблено науково-методичний підхід до оцінки відповідності системи охорони здоров’я регіонів України (Вінницька, Одеська, Рівненська, Львівська та Харківська області, які мають найкращі індикатори досягнення Цілі 3 Цілей сталого розвитку в Центральній, Південній, Північній, Західній та Східній Україні, відповідно) Цілям сталого розвитку за показниками (кількість зареєстрованих випадків ВІЛ-інфекції, рівень захворюваності на активний туберкульоз, кількість виписаних електронних рецептів, кількість проведених щеплень проти COVID-19, обсяг додаткових дотацій місцевим бюджетам, видатків медичним закладам, що уклали договір з НСЗУ, рівень забезпеченості практикуючими лікарями на 10 тис. населення, середня кількість декларацій на одного лікаря). За результатами оцінювання регіонів за допомогою пелюсткової діаграми в полярній системі координат визначено пріоритети розвитку регіонів, що досліджувались. У четвертому розділі роботи на основі дослідження міжнародного досвіду впровадження державних електронних послуг у систему охорони здоров’я (Австралія, Аргентина, Бразилія, Канада, Колумбія, Корея, Мексика, США), європейського досвіду надання електронних послуг (Великобританія, Естонія, Іспанія, Італія, Мальта, Німеччина, Норвегія, Румунія, Фінляндія, Франція, Швейцарія, Швеція) визначено напрями реалізації механізму надання електронних послуг в Україні. Реалізація зазначених напрямів з використанням інструментів публічних механізмів (регуляторні, фінансові, технічна інфраструктура) дасть змогу підвищити ефективність електронних форм взаємодії держави та громадян і забезпечити розвиток цифрового здоров’я в Україні.У п’ятому розділі доведено необхідність трансформацій у механізмах публічного управління щодо кадрового забезпечення сфери охорони здоров’я за умов дотримання міжнародних стандартів. Визначено напрями вдосконалення кадрового забезпечення у сфері охорони здоров’я відповідно до ключових пріоритетів трансформації та стратегічні механізми кадрового забезпечення у сфері охорони здоров’я, які потребують трансформації на найближчу перспективу. На основі узагальнення нормативно-правового, інституційного, фінансового механізму забезпечення реформ та трансформацій у медичній галузі сформовано стратегічну матрицю реалізації публічних механізмів трансформації та розвитку сфери охорони здоров’я України, яка включає аналіз таких елементів, як цілі, завдання, принципи, інструменти на міжнародному, національному, регіональному рівнях. Визначено спільні та відмінні риси кожного компонента на відповідному рівні. В умовах пандемії та оголошеного в Україні воєнного стану для досягнення Цілей сталого розвитку визначено напрями реалізації публічних механізмів трансформації у сфері охорони здоров’я України: нормативно-правового, інституційного, фінансового, кадрового механізму забезпечення, механізму надання електронних послуг, упровадження яких дасть змогу забезпечити позитивні результати у напрямі досягнення ЦСР як на рівні регіонів України, так і на світовому ринку.

The dissertation substantiates theoretical and methodological approaches and develops scientific and practical recommendations for the implementation of public mechanisms of transformations and development of healthcare in Ukraine (regulatory-legal, institutional, financial, personnel mechanisms, the mechanism for providing electronic services) in the conditions of sustainable development and modern challenges . The first chapter summarizes the mechanisms of public administration in the field of health care, which are implemented in foreign practice (financial, informational, institutional, organizational mechanisms) and domestic realities (institutional, economic, organizational, legal, financial, informational, personnel, the mechanism of state private partnership). The basic mechanism for the implementation of public policy in the specified field (institutional, organizational, economic, personnel, financial, informational, legal, public-private partnership) has been defined, taking into account global challenges, pandemic exacerbation and European integration agreements, the implementation of which should be ensured by such tools of public transformations as blockchain technology, information and communication tools, in accordance with the specified directions: effective and affordable health care system, improvement of the quality and safety of medical care, development of primary medical care, combination of public-private support and medical insurance. The second chapter of the work summarizes the methodological approaches to the classification of legislation in the field of health care, the legal framework for reforming the health care sector of Ukraine. A mechanism of regulatory and legal support for the development of health care is proposed, which includes international, national and regional levels. The principles of implementation of the mechanism of regulatory and legal support for the development of health care and the main strategic vectors of ensuring high-quality and effective state policy of Ukraine in the field of health care have been determined, which will make it possible to adapt the program documents of the field of health care to the Sustainable Development Goals. The institutional mechanism of state management of the health care system has been improved based on the definition of the main functions, tasks, organization of activities of international, national and regional institutes of state policy, instruments (normative-legal, financial, economic, information-communication, research and development, educational and enlightenment) and methods (organizational, economic, psychological, special, administrative, digital technologies) of their implementation at the central and regional levels of management. The study of financial support for the medical industry in the conditions of the pandemic made it possible to highlight the key problems of financing the industry. In order to increase the efficiency of the use of budgetary resources at various levels of state administration and ensure the sustainable development of the health care sector, the directions of transformation of public financial mechanisms for the provision of health care are highlighted, the implementation of which is possible in accordance with the principles of universality, justice, transparency, accountability, efficiency, free choice, supplier competition, subsidiarity and financial protection. In the third chapter, the systematization of the international experience of providing health care systems based on indicators (public health, access to medicines, provision of health care services and financing, personnel resources of the health care system) and the level of development of the health care system according to international ratings, proves that the strategic goals of the functioning of all health care systems are to ensure a capable public health system, accessibility and quality of medical services. A comparative analysis of the results of the implementation of public mechanisms in the field of health care in the context of a pandemic in Romania, Moldova, Hungary, Slovakia, Poland, and Ukraine made it possible to determine risk factors in the field of health care and key priorities in the field of health care. Adapted indicators of achievement of Goal 3 of the Sustainable Development Goals in Ukraine, regulated in the National Report "Sustainable Development Goals: Ukraine for the period until 2030", were predicted using Excel statistical functions based on linear regression. The need to implement public mechanisms for the transformation of the health care sector that meet the challenges of modern times has been proven. Taking into account the fundamentals of the strategy of reforming the medical industry, strategic changes in the government's activities in the context of the pandemic and the full-scale military invasion of the Russian Federation on the territory of Ukraine, the transformational directions of public mechanisms for the development of the health care system in the context of reform, pandemic and martial law have been determined, the implementation of which will ensure the achievement of sustainable development sphere of health care of Ukraine. An algorithm of actions to ensure a capable public health system is proposed. In the context of the transformation of public mechanisms for the regulation of health care at the local level, the priority areas for involving the community in the transformation of public mechanisms for the regulation of health care in the conditions of decentralization of Ukraine are highlighted. A scientific and methodological approach was developed to assess the compliance of the health care system of the regions of Ukraine (Vinnytsia, Odesa, Rivne, Lviv and Kharkiv regions, which have the best indicators of achieving Goal 3of the Sustainable Development Goals in Central, Southern, Northern, Western and Eastern Ukraine, respectively) for the purposes of sustainable development by indicators (the number of registered cases of HIV infection, the incidence rate of active tuberculosis, the number of issued electronic prescriptions, the number of vaccinations against COVID-19, the volume of additional subsidies to local budgets, expenditures to medical institutions that have signed a contract with the National Health Service, the level provision of practicing physicians per 10,000 population, average number of declarations per physician). According to the results of the evaluation of the regions using a petal diagram in the polar coordinate system, the development priorities of the studied regions were determined. In the fourth chapter of the work, based on the study of the international experience of introducing public electronic services into the health care system (Australia, Argentina, Brazil, Canada, Colombia, Korea, Mexico, the USA), the European experience of providing electronic services (Great Britain, Estonia, Spain, Italy , Malta, Germany, Norway, Romania, Finland, France, Switzerland, Sweden) defined directions for the implementation of the mechanism for providing electronic services in Ukraine. The implementation of the specified directions using the tools of public mechanisms (regulatory, financial, technical infrastructure) will make it possible to increase the effectiveness of electronic forms of interaction between the state and citizens and ensure the development of digital health in Ukraine. In the fifth chapter, the need for transformations in the public management mechanisms with regard to personnel support in the health care sector under the conditions of compliance with international standards is proven. The areas of improvement of human resources in the field of health care in accordance with the key priorities of transformation and strategic mechanisms of human resources in the field of health care, which require transformation in the near future, have been determined. Based on the generalization of the regulatory, institutional, and financial mechanism for ensuring reforms and transformations in the medical field, a strategic matrix for the implementation of public mechanisms for transformation and development of the health care sector of Ukraine was formed, which includes an analysis of such elements as goals, tasks, principles, tools at the international level, national level, regional levels. The common and distinctive features of each component at the corresponding level are determined. In the conditions of the pandemic and the declared state of war in Ukraine, in order to achieve the Sustainable Development Goals, the directions for the implementation of public transformation mechanisms in the sphere of health care of Ukraine have been determined: regulatory, legal, institutional, financial, personnel support mechanism, the mechanism for the provision of electronic services, the implementation of which will make it possible to ensure positive results in the direction of achieving the SDGs both at the level of the regions of Ukraine and on the world market.
