Трансформації системи охорони здоров'я в умовах децентралізації України

Abstract

Статтю присвячено дослідженню поточних трансформації у сфері охорони здоров'я в умовах децентралізації України для впровадження подальших механізмів реформування цієї галузі. Метою статті є визначення основних напрямів трансформації системи охорони здоров'я в умовах децентралізації України для подальшого удосконалення та розвитку сфери охорони здоров'я. У статті доведено, що рівень забезпечення організаційних, фінансових, інформаційних, кадрових, технічних, технологічних та інших ресурсних потреб системи охорони здоров'я не є рівномірним за регіонами, особливо на сучасному етапі трансформації галузі. Тому визначено проблеми системи трансформації системи охорони здоров'я на регіональному рівні, а також виявлено напрями змін, які відбуваються у сфері охорони здоров'я в умовах децентралізації. Обгрунтовано необхідність створення госпітальних округів та проведення перепрофілювання закладів вторинної медичної допомоги — як головних процесів трансформацій у сфері охорони здоров'я на регіональному та місцеву рівнях. Визначено мета та принципи трансформації госпітальної мережі закладів охорони здоров'я в Україні, а також напрями— взаємодії зацікавлених сторін у формуванні ефективних громад щодо розвитку ефективних госпітальних округів. У висновках визначено основні напрями трансформації системи охорони здоров'я в умовах децентралізації України для подальшого удосконалення та розвитку сфери охорони здоров'я. The article is devoted to the study of current transformations in the field of health care in the conditions of decentralization of Ukraine for the implementation of further mechanisms for reforming this sector. The purpose of the article is to determine the main directions of transformation of the health care system in the conditions of decentralization of Ukraine for further improvement and development of the health care sphere. The article proves that the level of organizational, financial, informational, personnel, technical, technological and other resource needs of the health care system is not uniform by region, especially at the current stage of transformation of the industry. Therefore, the problems of the health care system transformation system at the regional level are identified, as well as the directions of changes in the health care sector in the context of decentralization (transition of powers to the community level, the need for community cooperation for common goals, exclusion of budget revenues and powers of the district budget). The need to create hospital districts and restructure secondary health care facilities as the main processes of transformation in the field of health care at the regional and local levels is substantiated. The purpose and principles of transformation of the hospital network of health care institutions in Ukraine (availability, safety and quality of medical care, decentralization of less complex medical services, differentiation of services depending on the complexity of cases and risk management, concentration of more specialized medical care) and directions interaction of stakeholders in the formation of effective communities for the development of effective hospital districts. The conclusions identify the main directions of transformation of the health care system in the context of decentralization of Ukraine (introduction of key performance indicators for heads of health care institutions; formation of an effective system for monitoring the provision of medical services under the medical guarantee program; provision of emergency medical care, specialized medical care, improvement of the electronic health care system, at all levels, development of hospital districts) for further improvement and development of the health care sector.
