Державна політика в галузі охорони здоров'я України : необхідність комплексності і системності реалізації

Abstract

Систему охорони здоров'я досліджено у широкому розумінні як модернізацію системи управління всіх галузей, пов'язаних із діяльністю системи охорони здоров'я, що залежить від ефективності структурної реорганізації системи відповідно до потреб населення в різних видах медичної допомоги. Саме ефективна й доступна система охорони здоров'я визначається Урядом України як сучасне бачення системи охорони здоров'я після проведення реформи, що відповідає потребам населення України. Досліджено особливості впровадження державної політики з охорони здоров'я в Україні та перспективи її розвитку в умовах системних змін з урахуванням необхідності комплексності і системності реалізації основних її засад. Акцентовано увагу на ключових напрямах перетворень у системі охорони здоров'я України. Встановлено, що модернізація системи охорони здоров'я має включати такі основні напрями: зміна мережі установ охорони здоров'я; зміна системи фінансування охорони здоров'я; вдосконалення системи контролю якості (запровадження стимулів і самого контролю якості надаваних послуг). Проаналізовано основні показники ефективної державної політики в галузі охорони здоров'я. Доведено, що ефективне функціонування системи охорони здоров'я визначається такими основними системоутворюючими факторами: вдосконалення організаційно-правового забезпечення; розвиток інфраструктури та ресурсного забезпечення охорони здоров'я; наявність достатньої кількості підготовлених медичних кадрів. Правові засади реформування системи охорони здоров'я мають передбачати нові впровадження законодавства та зміни всіх функцій системи охорони здоров'я: управління цією системою, забезпечення її відповідними ресурсами, фінансування та надання якісних медичних послуг. Акцентовано увагу на необхідності проведення конкретних управлінських та організаційних заходів, спрямованих на реформування системи охорони здоров'я України, що повинні стосуватися оновлення механізмів державного управління галуззю, а також наукового обгрунтування та практичного впровадження нових методів управління, які б концентрували зусилля органів державної влади на розв'язання протиріч і усунення невідповідностей між фактичними потребами населення в медичній допомозі та послугах і реальними можливостями системи охорони здоров'я щодо якісного задоволення цих потреб в умовах трансформаційних перетворень. The health care system is studied in a broad sense as the modernization of the management system of all spheres related to the health system, which depends on the effectiveness of structural reorganization of the system in accordance with the needs of the population in various health care types. The most effective and affordable health care system is defined by the Government of Ukraine as a modern vision of the health care system after the reform that meets the needs of the population of Ukraine. The peculiarities of the implementation of the state policy on health care in Ukraine and the prospects of its development in the conditions of systemic changes taking into account the need for comprehensiveness and systematic implementation of its basic principles are studied. Emphasis is placed on key areas of change in the health care system of Ukraine. It is established that the modernization of the health care system should include the following main directions: change of the network of health care institutions; change in the health care financing system; improving the quality control system (introduction of incentives and the quality control of provided services). The main indicators of effective state policy in the health care sphere are analyzed. It has been proven that the effective functioning of the health care system is determined by the following main systemforming factors: improving organizational and legal support; development of health infrastructure and resource provision of health care; availability of a sufficient number of trained medical personnel. Moreover, the legal framework for health care reform should include new legislation and changes in all functions of the health care system: management of this system, providing it with appropriate resources, financing and providing quality medical services. Emphasis is placed on the need for specific management and organizational measures aimed at reforming the health care system of Ukraine, which should relate to updating the mechanisms of public administration in the industry, as well as scientific substantiation and practical implementation of new management methods that would concentrate the efforts of government authorities to resolve contradictions and eliminate discrepancies between the actual needs of the population in health care and services and the real capabilities of the health care system to meet these needs in the transformation conditions.
